Introductory Care Package

Our Introductory Care Package is designed to get a complete picture of your health and medications, so we can create a personalized plan just for you. All patients begin here with a chance to continue on to our membership options after completion. This package is 2 months of care (starting from the date of the first introductory visit).

  • Two visits within the same week:

    • Visit 1: Gather your health history, medications, and daily routine

    • Visit 2: A personalized review of all your medications to make sure they’re safe, effective and easy to manage (comprehensive MTM review), provide recommendations, and create your care plan

  • One in-person follow-up visit at the patient’s home within the two month period.

    • Review how the care plan is working

    • Make any adjustments to medications, pillboxes, or routines

    • Answer any questions from the patient or caregiver

  • Unlimited Pharmacist Access Via Phone, Text & E-mail (Monday-Friday, 8am-5pm & Sat-Sun 9am-3pm). This is available for the two month period (starting from the first introductory meeting).

    This package gives you peace of mind, personalized guidance, and a solid foundation for ongoing care if you choose to continue with a monthly membership.

Assurance Membership

Designed for seniors and caregivers who want continuous, personalized pharmacy support- all year long. Billed monthly. Available after completion of the Introductory Care Package.

  • One Monthly Home Visit

  • Unlimited Pharmacist Access Via Phone, Text & E-mail (Monday-Friday, 8am-5pm & Sat-Sun, 9am-3pm).

Guardian Membership

Designed for seniors and caregivers who want continuous, personalized pharmacy support that have an extensive medication list and need frequent medication reconciliation due to doctor visits/hospital admissions. Billed monthly. Available after completion of the Introductory Care Package.
